To Request A Name

If you would like to request the name of a lost child to remember please fill out the Name Request Form. If the date of the bar/bat mitzvah is less than 14 days away we will send your name via email.

The Memorial Sheet for your lost child will be sent by regular mail. Read the Student Invitation, Parent Guide, Mentor’s Guide, and Newsletters for more information on how to do this mitzvah.

To Participate

Preparation for bar or bat mitzvah can be time-consuming. Participation in Remember Us is simple.

If you are a congregation, contact us by email. Tell us the number of boys and girls who may be participating this year. We will send the names of children to be remembered and program materials.

If you are a family, tell us if you want a name to remember for a boy or a girl.
